com Cigna HealthCare will announce today that it is partnering with Medical Clinic of North Texas to launch a pilot program aimed at improving quality and reducing health care costs of patients with chronic conditions. To support the “patient-centered medical home” program which begins Tuesday Cigna is hiring a care coordinator to help the clinic’s 82 doctors find cost-effective ways to treat Cigna’s 10000 patients. An estimated 10 percent of those patients have chronic conditions such as diabetes and have a higher risk of being admitted to an emergency room. For people requiring more follow-ups than an average patient the coordinator will work to ensure that the doctor’s orders are clearly communicated and that the patients comply. The medical home program also means the patients’ records will be shared electronically among their primary doctor specialists and pharmacists.
